Full-Time RN (Evening): $3,000 sign-on bonus*

Full-Time LPN (Evening): $3,000 sign-on bonus*

*In order to qualify for the sign-on bonus the stakeholder must be hired as a full time evening R.N./LPN. The first payment of $1000.00 will be made after the Stakeholder completes 90 days of employment with satisfactory or above performance. The second payment of $1000.00 will be made after the Stakeholder completes 180 days of satisfactory or above performance. The third payment of $1000.00 will be made after the Stakeholder completes a year of employment with satisfactory or above performance. The sign-on bonus is not part of base pay but is considered taxable income. The Stakeholder understands that the bonus is forfeited if the Stakeholder’s employment should terminate whether termination is voluntary or involuntary. Stakeholder must maintain a full time status for a period of one year. Payments are forfeited if the stakeholder reaches the second level of attendance coaching and counseling; the sixth absence within a Stakeholder’s first anniversary year – written, or if their performance is not satisfactory.
